Reading Intervention Summary Report
I have been working on a report template for literacy interventions, I wanted something that would reflect the following 5 areas:
- Phonological Awareness
- Phonics
- Fluency
- Vocabulary
- Comprehension
I also wanted to be able to have recommendations for class to embed as well as an opportunity to share more information that might support teachers further. See below for the report template that I have managed to create. It is for intervention/progress monitoring. However, I think this could be adapted for assessment purposes too.
The text in PURPLE gives suggestions/advice for completing the section. The recommendations and further resources can also be edited/deleted as appropriate.
Before you download, a few disclaimers:
- The recommendations are not an exhaustive list, and are not necessarily relevant for every learner.

- This report is not to be used in place of assessment or monitoring from a qualified professional.
- The purpose of the references/footnotes are for your reference. But this may not be appropriate to share with other professionals, families or stakeholders purely from an accessibility standpoint.
